Please note schedules may change due to operational needs.</p> <p><strong>Compensation:</strong></p> BAND J Range: $39.10 - $56.05
Responsibilities: <ul> <li>Manages telephone Triage Calls.</li> <li>Eliminates barriers to patient's access to health care services and facilitates continuity of care/care coordination.</li> <li>Acts as a liaison, advocate and point of contact to patient/family members by helping them access the best healthcare outcomes from doctors, hospitals and others who provide their required services. .</li> <li>Acts as advisor/educator by providing emotional support, counseling, clinical education, resources and expert guidance to patients and families to promote their ability to understand and meaningfully participate in the healthcare process and personal decision-making.</li> <li>Provides nursing assessment including assessments of physical, psychological, social, cultural and spiritual needs.</li> <li>Provides full scope of nursing knowledge and skills in Oncology.</li> <li>Demonstrates strong evidenced based and family centered approach to holistic care for all patients.</li> <li>Demonstrates strong clinical skills and expertise to independently follow physician orders to implement care and treatments and provide nursing care to ill patients and complex and frail elderly patients.</li> <li>Works with the interprofessional team to provide a holistic plan of care, communicate with the patient and family/caregiver, explore patient needs and plan with them to transition back to the community seamlessly.</li> <li>Uses nursing judgment and expertise to organize sequencing of community supports, physician assessments, diagnostic tests and procedures where required.</li> <li>Keeps abreast of the patient&rsquo;s response to community care providers, interventions and modifying the care plan accordingly in communication with the physician.</li> <li>Provides a comprehensive report when transferring accountability of a patient&rsquo;s care to another health care professional, community agency and/or facility.</li> <li>Establishes a caring and therapeutic relationship with patients; demonstrate trust, respect, honesty and provide counseling as required.</li> <li>Expedites access to services and resources to improve continuity and coordination of care between hospital and home.</li> <li>Empowers patients and their caregivers through the development of trusting relationships, and through providing information and education to facilitate decision making in order to enhance the patient&rsquo;s choice of the best alternatives for supportive care in the community</li> </ul>
Requirements: <br /> <ul> <li>Current Certificate of Registration with the College of Nurses with BScN required</li> <li>Current BCLS certification</li> <li>Minimum of 5 years of experience in Oncology nursing or an Oncology clinic setting</li> <li>Completion of chemotherapy administration, deSouza Chemotherapy/Biotherapy Administration Certification</li> <li>CNA Oncology Certification preferred, and/or a commitment to complete within 2 years of hire</li> <li>RNAO membership and Active member of Canadian Association of Nurses in Oncology (CANO) is preferred</li> <li>Excellent physical assessment and patient education skills</li> <li>Working knowledge of computer technology</li> <li>Demonstrated commitment to a patient/family centred care environment</li> <li>Demonstrated ability to multi task in a fast paced environment</li> <li>Demonstrated excellent critical thinking and problem solving skills</li> <li>Demonstrated ability to work effectively within an inter professional team as well as independently</li> <li>Well developed organization, communication, interpersonal and teaching skills</li> <li>Evidence of ongoing professional development, continuing education and leadership experience</li> <li>Demonstrated good attendance record with the ability to maintain this same standard</li> <li>Demonstrated commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required</li> </ul>
