Part of a larger integrated primary health care team at OVH, the NP will provide virtual patient consultation and assessments&nbsp;within the scope of the primary care NP for patients of all ages and demographics who book an appointment.<br /><br /></p> <p><strong>Work schedule:</strong></p> <p>The successful candidate will be required to work primarily 8-hour shifts evenings from 1pm to 9pm Monday through Sunday. Responsibilities: <ul> <li>This role involves a strong leadership component with specific focus on initiative, communication, integration of care and follow up to help mitigate barriers to access for care and help deter ED visits.&nbsp;</li> <li>This role will include medical assessment, treatment and evaluation, education and health promotion, disease and injury prevention, acute medical treatment, and ability to make connections to the next level of care or follow up as required.&nbsp;</li> <li>The NP will need advanced knowledge and decision-making skills to provide a comprehensive health-focused approach in meeting the needs of patients.&nbsp;</li> <li>The NP can work collaboratively with a team to determine next steps for patients in ensuring appropriate transitions in care and follow up.</li> <li>The NP will work collaboratively with the hospital physicians and community partners caring for patients in the community.&nbsp;&nbsp;</li> <li>The NP will work closely with the Physician Lead for EYRND OHT and its associates in establishing clinical pathways and escalation processes.</li> </ul>
Requirements: <ul> <li>Master&rsquo;s Degree in a related field or working toward.</li> <li>Current registration from the College of Nurses of Ontario as an RN (EC) - Nurse Practitioner-Primary Health Care, all ages (PHCNP).</li> <li>Demonstrated current expertise in primary care an asset.</li> <li>Membership in the Registered Nurses&rsquo; Association of Ontario &amp; Nurse Practitioner Association of Ontario, preferred.</li> <li>Advanced clinical knowledge and implementation of evidenced based practice.</li> <li>Superior interpersonal, coaching, and leadership skills.</li> <li>Excellent decision making, problem recognition and problem-solving skills.</li> <li>Demonstrated commitment to continuing professional development.</li> <li>A strong desire and capacity to promote professional practice and excellence in evidence-based, patient-centered care.</li> <li>Ability and willingness to work remotely from home.</li> <li>Ability and willingness to work between the hours of 1300 to 2100 Monday through Sunday on rotating schedules.</li> <li>Demonstrated competency in the following areas: health assessment and diagnosis; health care management and therapeutic intervention; health promotion and prevention of illness, injury and complications in the care of primary care patients including frail seniors.</li> <li>Demonstrates the competencies to autonomously diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures within the RN(EC) legislated scope of practice and designated medical directives, as appropriate.</li> <li>Advanced knowledge, skill and decision making and expertise in primary care.</li> <li>Facilitates application of relevant information to make effective judgments and decisions to lead the management of all patient types.</li> <li>Effective communication skills, engages others, and is a reflective listener and learner.</li> <li>Evidence of program design, resource development and project management skills.</li> <li>Self-directed, works independently and able to achieve outcomes.</li> <li>Proven analytical thinking and proactive problem-solving and decision-making skills.</li> <li>Excellent report writing skills.</li> <li>Proficient with computer programs including Word, PowerPoint, Outlook and Virtual Platform Zoom Health.</li> <li>Working knowledge of the College of Nurses of Ontario standards of practice.</li> <li>Demonstrated good attendance and performance record with the ability to maintain these same standards.</li> <li>Demonstrated commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital-related policies, health and safety legislation and best practices and completing relevant mandatory education as required.</li> </ul> <br /><br /> <p><strong>Who we are:</strong></p> <p>Oak Valley Health is one of Ontario&rsquo;s leading community healthcare organizations.&nbsp;Across our two sites (Markham and Uxbridge) and Reactivation Care Centre (RCC), we provide high quality, patient-centred care to more than 468,000 patients each year.
