Overview: <p>This position is responsible for reviewing active and closed Hospice and Home Care charts and records, for identifying and reporting problems, for assisting with root cause analysis and for assisting with implementing corrective changes.<br /><br /></p> <p>This position is a key clinical leadership role within the organization, with the ability to make an impact and offers a significant personal and professional opportunity to join a world-renowned health care organization. Responsibilities: <ul> <li>Regularly audit active and closed patient electronic records</li> <li>Compose comprehensive report of audit findings for dissemination to agency staff</li> <li>Review each admission for completeness and accuracy - request staff to make corrections as needed</li> <li>Record key data elements from admission review for Structural indicators</li> <li>Develop new Structural indicators and monitoring tools as needed</li> <li>Prepare quarterly reports for QAPI committee and annual submission to governmental agency (CMS)</li> <li>Update QAPI manuals quarterly</li> <li>Review quarterly results of the CAHPS Hospice Survey, and review individual surveys in order to institute corrective actions</li> <li>Interface with Administrative staff to identify problems, find the root cause of problems and assist with resolution intervention</li> <li>Interface with staff to train them on medical record software when data issues emerge</li> <li>Maintain medical record software reference material and share with others when further teaching is requested or necessary</li> <li>Attends Hospice IDG and Home Care meetings as required by agency policy</li> <li>Functions as clinical resource for Parker personnel</li> <li>Serves on Performance Improvement Committee</li> </ul>
Requirements: <ul> <li>Current New York State &nbsp;RN license</li> <li>3-5 years progressive experience in Quality Assurance</li> <li>Hospice and Home Care experience required</li> <li>Ability to use EMR-knowledge of Netsmart a plus</li> <li>Current Driver&rsquo;s License</li> </ul>
