<p>Parker Jewish Institute is seeking a&nbsp;Full Time Social Worker-Family Caregiver Coordinator&nbsp;to join a five-year grant-funded project under the Long Island Caregiver Support Initiative.</p>

Responsibilities: <ul> <li>Responsible for conducting intake assessments and providing referrals&nbsp; in relation to the program&nbsp;</li> <li>Monitoring, tracking, follow-up and analysis of family and community services</li> <li>Monitoring and analysis of attendance and outcomes</li> <li>Implementing innovative caregiver support services, including support groups, education and training programs, caregiver wellness programs and join enrichment opportunities</li> <li>Providing information regarding services and benefits available, and, where appropriate, assisting clients in accessing them</li> <li>Marketing and outreach of the program; liaising with relevant community based programs&nbsp;</li> </ul>
Requirements: <ul> <li>Master&rsquo;s Degree in Social Work or a related field</li> <li>License to practice Social Work in New York State strongly preferred</li> <li>At least two years of relevant coordination, case management or outreach experience</li> <li>Prior experience working with the geriatric population</li> <li>Advanced proficiency in Microsoft Office suite including Excel, PowerPoint and Access&nbsp;</li> </ul>
