Overview: <p>Parker Jewish Institute Hospice and Home Health Care offers the finest in home health and hospice services from an interdisciplinary team of health care professionals. Parker Jewish Institute for Health Care and Rehabilitation is a not-for-profit adult health care system comprised of multiple health services servicing the Metropolitan New York Area, including Long Island, Brooklyn, Queens, Bronx, Manhattan, and Westchester.</p> <p>This position requires the full understanding and active participation in fulfilling the mission of Parker Hospice and Home Care is to deliver exceptional care and to enhance the quality of life of our patients.</p> <p>The Field Nurse is responsible for the direct care and documented coordination of patients of the Hospice and Home Care programs. Under the direction of the Nursing Supervisor, the Field Nurse assures patient eligibility to continue on the program.&nbsp;<br /><br /><br /></p>
Responsibilities: <div> <p><strong><u>&nbsp;</u></strong></p> </div> <ul> <li>Provides professional skilled nursing service in patient's home, based on theoretical nursing concepts of practice and the Policy and Procedure of the&nbsp;Agency</li> <li>Promotes and maintains the health of patients in the community through teaching, counseling, and providing direct nursing care</li> <li>Assures compliance with Federal and State regulations</li> <li>Oversees patients in the community by offering professional supervision, health teaching, and hands on care in order to maintain patient safety in their home environment</li> <li>Ensures patient information is kept current in Netsmart, the Electronic Medical Record</li> </ul>
Requirements: <p><strong><u>&nbsp;</u></strong></p> <ul> <li>Current New York State license as a Registered Nurse (RN)</li> <li>BSN in Nursing preferred</li> <li>One year of home care, hospice or community RN experience preferred</li> <li>Knowledge of care coordination a plus</li> <li>Able to use computer technology in the field</li> <li>Access to reliable transportation</li> </ul>
