Responsibilities: <div> <p><strong><u>&nbsp;</u></strong></p> </div> <ul> <li>Provides professional skilled nursing service in patient's home, based on theoretical nursing concepts of practice and the Policy and Procedure of the Hospice Program&nbsp;</li> <li>Promotes and maintains the health of patients in the community through teaching, counseling, and providing direct nursing care</li> <li>Assures compliance with Federal and State regulations</li> <li>Oversees patients in the community by offering professional supervision, health teaching, and hands on care in order to maintain patient safety in their home environment</li> <li>Ensures patient information is kept current in Netsmart, the Electronic Medical Record</li> </ul>
Requirements: <p><strong><u>&nbsp;</u></strong></p> <ul> <li>Current New York State license as a Registered Nurse (RN)</li> <li>BSN in Nursing preferred</li> <li>One year of home care, hospice or community RN experience strongly preferred</li> <li>Knowledge of care coordination a plus</li> <li>Able to use computer technology in the field</li> <li>Access to reliable transportation</li> </ul>
