In this role, you&rsquo;ll assess resident health status, develop and update care plans, communicate with staff and families, and ensure compliance with all regulatory standards. You&rsquo;ll foster a safe, compassionate, and engaging environment while supporting the professional growth of the care team.</p>
Responsibilities: <ul> <li>Assess residents&rsquo; health upon admission and ongoing changes in condition, and develop or update individualized care plans.</li> <li>Communicate resident needs and changes to the Director, Health and Wellness, staff, and families.</li> <li>Participate in hands-on care and perform specific nursing tasks per residents&rsquo; care plans.</li> <li>Train and support care staff to meet continuing education and competency requirements.</li> <li>Transcribe and review physician orders; monitor medication administration and perform MAR reviews.</li> <li>Ensure compliance with state, federal, and local regulations, including HIPAA, OSHA, and Bloodborne Pathogen Guidelines.</li> <li>Participate in Continuous Quality Improvement (CQI) initiatives and report issues to leadership.</li> <li>Work safely and model safe practices for staff; correct unsafe actions as needed.</li> <li>Foster teamwork, show initiative, and positively impact coworkers, residents, and visitors.</li> <li>Report any elder maltreatment or abuse in accordance with policy.</li> <li>Perform other duties as assigned.</li> </ul>
Requirements: <ul> <li>Associate or Bachelor&rsquo;s Degree in Nursing preferred.</li> <li>Valid State Nursing License (RN or LPN/LVN); required certifications as mandated by state.</li> <li>Minimum 1 year of nursing or caregiving experience, preferably in Assisted Living or Memory Care.</li> <li>Experience with Microsoft Word and Excel; familiarity with human resource rules a plus.</li> <li>Strong clinical assessment skills, conflict resolution, and problem-solving abilities.</li> <li>Excellent written, verbal, and interpersonal communication skills.</li> <li>Compassion for and desire to work with the elderly.</li> <li>Self-motivated, organized, detail-oriented, and able to work independently.</li> <li>Pleasant, professional, and personable demeanor.</li> </ul>
