You will provide care to clients with intellectual and developmental disabilities in their own home. The Arc of San Diego operates nine (9) ICF-DD-H residential facilities across San Diego County.&nbsp;</div> <div> <div>&nbsp;</div> The hours for this role are Monday &ndash; Friday 8:30 AM &ndash; 5:00 PM.</div> <div>&nbsp;</div> <div>The pay for this position is $47 to $52 per hour depending on experience.&nbsp;</div> <div>&nbsp;</div> <strong>Who you are:</strong><br /> <ul> <li>Comfortable providing full care coordination</li> <li>A strong client advocate</li> <li>Have active observation skills</li> <li>A critical thinker</li> <li>Observe the rights and best interest of adults with intellectual/developmental disabilities</li> </ul>
Responsibilities: <ul> <li> <div>Physical and nutrition assessments</div> </li> <li> <div>Developing individual health care plans</div> </li> <li> <div>Providing case management support</div> </li> <li> <div>Oversight for clients' specialty appointments</div> </li> <li> <div>Pharmacy and medication review</div> </li> <li> <div>Psychiatrist and mental health review</div> </li> <li>Scheduling and attending coordinated client service planning meetings</li> </ul>
Requirements: <ul> <li>CA license as Registered Nurse</li> <li>6 months hands on experience working with people with intellectual and/or developmental disabilities</li> <li>Valid CA Driver&rsquo;s License with clean driving record</li> </ul> <div><strong>We&rsquo;d love it if you:</strong></div> <ul> <li>Have experience using electronic case management systems (Therap)</li> <li>Previously provided in-home care in individual or group setting</li> <li>Experience working with people with intellectual and/or developmental disabilities</li> </ul>
