Overview: <p style="font-weight: 400;"><span data-darkreader-inline-color="">Touchmark at All Saints is Sioux Falls's exclusive resort-style retirement community,&nbsp;enriching the lives of residents and their families through five-star hospitality, services, and amenities.<br /></span><span data-darkreader-inline-color=""><br />We are currently seeking the right person to join our team as a full-time&nbsp;<strong>LPN&nbsp;</strong>in our Health Services (to include Memory Care and Assisted Living)&nbsp;neighborhoods.&nbsp;</span></p> <p style="font-weight: 400;"><span data-darkreader-inline-color="">As an LPN, you will enrich lives every day by utilizing your education and experience to ensure residents in our Assisted Living and Memory Care neighborhoods receive the highest level of dignity and care. Responsibilities: <ul style="font-weight: 400;"> <li><span data-darkreader-inline-color="">Ensure that you are adhering and exemplifying the Touchmark Gold Standards in every aspect of your position and all to those that you serve, lead and work alongside.</span></li> <li><span data-darkreader-inline-color="">Assess nursing care given by self and non-professional staff members.&nbsp;</span></li> <li><span data-darkreader-inline-color="">Identify problems and/or need for improvement and communicate this information to the Health Services Director;</span></li> <li><span data-darkreader-inline-color="">Assess the health status of residents and develop and review the individual service plans as required in state regulations;</span></li> <li><span data-darkreader-inline-color="">Assist in completing monthly summaries and other documentation required in all of the health services and all levels of care.</span></li> <li><span data-darkreader-inline-color="">Collect data from residents, families, significant others, health care providers, individuals, and/or agencies in the community that is pertinent for resident care;</span></li> <li><span data-darkreader-inline-color="">Organize assessment data so that it is accurate, complete and accessible, and so that it remains confidential;&nbsp;&nbsp;&nbsp;</span></li> <li><span data-darkreader-inline-color="">Perform all clinical duties within the RN scope of practice and supervise all delegated nursing acts in accordance with the Board of Nursing standards of practice for registered nurses;&nbsp;</span></li> <li><span data-darkreader-inline-color="">Communicate assessment data in an orderly fashion by recording, updating, and communicating among the health team daily and revisiting as appropriate;</span></li> <li><span data-darkreader-inline-color="">Suggest ways to solve problems and make improvements;</span></li> <li><span data-darkreader-inline-color="">Formulate assigned residents&rsquo; care plans;</span></li> <li><span data-darkreader-inline-color="">Identify resident problems and communicate to Health Services Director;</span></li> <li><span data-darkreader-inline-color="">Work in collaboration with Retirement Counselors if the Health Services Director is not available to assist during tours and explain Touchmark programming.</span></li> </ul> <p style="font-weight: 400;"><span data-darkreader-inline-color="">This is a Monday &ndash; Friday 8 am &ndash; 5 pm position with a weekend rotation.</span></p>
Requirements: <p style="font-weight: 400;"><strong><span data-darkreader-inline-color="">REQUIRED QUALIFICATIONS:</span></strong></p> <ul> <li style="font-weight: 400;"><span data-darkreader-inline-color="">Current and unencumbered&nbsp;LPN license in the state of South Dakota;</span></li> <li style="font-weight: 400;"><span data-darkreader-inline-color="">Minimum 3 years&rsquo; experience working with seniors people who need assistance with daily living;</span></li> </ul> <p style="font-weight: 400;"><strong><span data-darkreader-inline-color="">ESSENTIAL FUNCTIONS:</span></strong></p> <ul style="font-weight: 400;"> <li><span data-darkreader-inline-color="">Ability to lead, manage, model, and make decisions in accordance with the Touchmark Values;</span></li> <li><span data-darkreader-inline-color="">Must be able to complete required paperwork and filing with a high degree of accuracy;</span></li> <li><span data-darkreader-inline-color="">Ability to observe the resident's behavior and communicate observations in a professional intelligent manner;</span></li> </ul>
