Responsibilities: <p style="font-weight: 400;">As a&nbsp;Nurse Practitioner<strong>&nbsp;</strong>with Touchmark on South Hill<strong>,</strong>&nbsp;you will:</p> <ul style="font-weight: 400;"> <li>Be responsible for providing an advanced level of nursing practice to patients and families, servings as a clinical resource for staff and interdisciplinary team</li> <li>Facilitate medical care for Touchmark at South Hill skilled nursing, assisted living and memory care patients</li> <li>Effectively evaluate, assess, diagnose patients&rsquo; conditions, and implement treatments in accordance with practice standards</li> <li>Communicate needs to patient/caregiver or other members of the health care team in a timely fashion</li> <li>Maintain system to address patient&rsquo;s on-going prescription needs to avoid lapses in medications</li> <li>Document and communicate orders regarding patient care needs in a timely fashion.</li> <li>Enjoy a flexible schedule</li> <li>Exhibit excellent customer service when responding to patient concerns in a timely manner.</li> <li>Function as a dependable team member demonstrating ability to collaborate and share knowledge</li> <li>Participate in evening and on-call rotations and respond promptly and courteously to incoming calls</li> </ul>
Requirements: <p style="font-weight: 400;">The successful candidate will:</p> <ul style="font-weight: 400;"> <li>Have a current Nurse Practitioner License in the state of Washington</li> <li>Be board-certified/eligible at time of employment</li> <li>Hold a current BLS for healthcare providers by the American Heart Association</li> <li>Have DEA, NPI and prescriptive authority&nbsp;&nbsp;&nbsp;&nbsp;</li> <li>Have a current driver's license, reliable transportation, and auto insurance</li> <li>Family Nurse Practitioner specialty in adults and geriatrics preferred &ndash; not required</li> <li>Have excellent leadership, interpersonal, written and verbal communication skills</li> <li>Be able to make decisions independently and be self-motivated&nbsp;</li> </ul>
