Full-Time

Overview: <strong>Touchmark Home Health</strong>&nbsp;has an opening for a Licensed Practical Nurse in our fast-paced agency. This position will work closely with our staff clinicians and medical offices in the community to ensure timely and appropriate care for our patients. This person will report to our Director of Professional Services.<br /><br />This is considered a full-time position and scheduled for approximately 30 hours a week with the potential for more hours depending on agency caseload. The position is Monday through Friday during the day and is eligible for full benefits and Paid Time Off (after the appropriate introductory period) as well as participation in our 401(k) and bonus programs.
Responsibilities: <strong>Position Requirements Include, but Are Not Limited to:</strong><br /> <ul> <li>Organizational and leadership skills that demonstrate the ability to plan, direct, supervise and assist with patient care and employee responsibilities;</li> <li>Patience and tact in relating to patients, family members, volunteers, visitors, physicians, and the general public;</li> <li>Commitment to confidentiality with respect to patient status and medical records;</li> <li>Ability to effectively document appropriately in client charts and communicate changes promptly with other team members.</li> <li>Ability to work well with co-workers, demonstrate integrity and express a positive attitude and sense of humor.</li> <li>Knowledge of medication systems and procedures which include: <ul> <li>ability to recognize and calculate a proper dosage of medications;</li> <li>ability to recognize and be familiar with abbreviations, symbols, and general names pertaining to medications which are commonly ordered and;</li> <li>ability to recognize reactions to medications which are commonly ordered;</li> </ul> </li> <li>Willingness to accept supervision;</li> </ul> <strong>Position Responsibilities Include, but Are Not Limited to:</strong><br /> <ul> <li>Communicate with physicians via phone/fax to assure patients receive care and services per service plan and health conditions are adequately monitored;</li> <li>Respond to patient calls and provide direction, following up with notifications and reporting;</li> <li>Assure that communication with physicians is followed up, review responses, and assure appropriate transcription of physician orders;</li> <li>Maintain resident's health records per professional standards and state regulatory requirements;</li> <li>Review patient medication lists for accuracy, therapeutic ranges, and guidelines.</li> <li>Assist with clinician scheduling based on the availability of orders while considering routing, other disciplines, and patient considerations.</li> <li>Respond to clinician requests for order reconciliation and follow-up, making sure that communication is prompt and documented appropriately between multiple disciplines.</li> <li>Participate in developing professionalism for self and co-workers;</li> <li>Utilize audit tools and quality improvement processes to assure high standards of service and regulatory compliance are maintained;</li> <li>Perform all other duties, as requested by the supervisor(s), with a positive attitude.</li> </ul>
Requirements: <strong>Successful Candidates will have:</strong><br /> <ul> <li>Licensed Practical Nurse degree from an accredited school of nursing;</li> <li>Current LPN license with the State of Washington;</li> <li>At least 2 years Care Coordination experience</li> <li>Medicare experience helpful</li> </ul>
