Overview: <strong>Touchmark at Fairway Village Home Health</strong>&nbsp;has an opening for a part-time/on-call Physical Therapist.&nbsp;<br /><br />As a Home Health Physical Therapist (PT) you will enrich the lives of our patients in the comfort of their own home in Clark County. You will be able to touch and improve the lives of patients and family members of patients every day.&nbsp;
Responsibilities: <ul> <li>Review physician referrals and create a personalized physical therapy treatment plan that sets realistic and achievable goals for patients in a home setting;</li> <li>Record and document patient condition, education, and physical progress;</li> <li>Work with interdisciplinary team to develop plan of care;</li> <li>Create and monitor PT treatment plans for physical therapy assistants;</li> <li>Work independently in patient homes using your good judgement and critical thinking skills to best serve the patients;</li> <li>Have transportation to patient homes throughout Spokane County.</li> </ul>
Requirements: <ul> <li>Current Physical Therapist (PT) License in good standing in Washington State</li> <li>Current CPR &amp; First Aid certifications</li> <li>Transportation to patient home, current drivers license, vehicle insurance, and complete and follow Touchmark Driving Policy.</li> </ul>
