Overview: <p>Volunteers of America Community Family Supports program is looking for dedicated and energetic individuals to join our team as a Mental Health Case Manager. Mental Health Targeted Case Management provides services supporting children and adults with mental health concerns. The goal of Case Management is to keep clients in their homes and community. Case Managers help coordinate services to support areas such as mental health, social, education, health, vocation, recreation, volunteer, advocacy, and transportation. This position is located in our New Hope office and serves primarily children and their families in the Hennepin County area.&nbsp;</p>
Responsibilities: <ul> <li>Provide services in the four core case management components-Assessment, Service plan Development, Referral and Linkage, and Monitoring and Coordination of Services</li> <li>Coordinate with a mental health professional to assure the completion of a mental health assessment of the child&rsquo;s and family&rsquo;s needs and to develop individual treatment plans.</li> <li>Develop and update the child or adult's individualized plan. Coordinate, monitor, and evaluate services that are outlined in the plan for appropriateness, progress, and continued service need.</li> <li>Coordinate services with a parent, child, and other members of the multi-disciplinary team.</li> <li>Develop and utilize available area resources and make appropriate referrals.</li> <li>Meet with the family and/or program participants as described in the plan and program policies. (* Currently Meetings are on-line due to Covid-19, however, will return to in-person in the future)</li> <li>Encourage and support parents in the parenting of their children.</li> <li>Keep necessary case notes documenting contact with program participants, family, and collateral contacts.</li> <li>Maintain program participant case files.</li> </ul>
