Overview: <p><a href="https://www.voamnwi.org/cesdm">Center for Excellence in Supported Decision Making | Volunteers of America (voamnwi.org)</a> is seeking a highly motivated, creative, licensed social worker who enjoys work-family life balance, and wants to experience the thrill of being part of systems change in Minnesota for people living with decisional capacity challenges, their families, and the professionals who serve them.&nbsp;</p> <p>This position is a key team member, working alongside Anita Raymond, Kathleen Carlson and Melissa Johnson:&nbsp; <strong><em>Lead Social Worker for Guardian Information Line</em></strong>.&nbsp; Primary focus of position is provision of extensive phone based, as well as opportunities for direct social work, with individuals, families, and professionals concerned about decision-making capacity and rights of adults living with disabilities across the adult life-span. &nbsp;Work includes education about Guardianship/Conservatorship law, Supported Decision Making, and less restrictive alternatives available to meet the needs of the person, which may include persons with cognitive disorders, chemical dependency, mental health issues or intellectual developmental disabilities for adults 18+. Join a supportive, collaborative team seeking systems change promoting supported decision making and guardianship reform, balancing safety with rights and dignity of risk.</p>
Responsibilities: <ul> <li>Conduct phone intake, person-centered assessments, formulate care plans based on assessment results to meet needs and wishes of persons served</li> <li>Promote value of Supported Decision Making in addressing client needs and vulnerabilities as a way to avoid unnecessary guardianship to families and professionals</li> <li>Team with VOA MN&rsquo;s Estate and Elder Law and private attorneys to facilitate appointment of surrogate or supported decision maker</li> <li>Prepare case to petition the court for guardianship/conservatorship when less restrictive options are not available</li> <li>Interact with attorneys, guardians, conservators, physicians and other health care professionals, county staff, professionals within the judicial system, family members and voluntary and involuntary program participants</li> <li>In conjunction with supervisor, complete activities in fulfillment of county contracts, including: Facilitate annual training for volunteer guardians/conservators; nomination of guardians/conservators for county petitions; conducting ongoing monitoring of activities and quality assurance of contracted volunteer guardian/conservators&rsquo; work</li> <li>In conjunction with team, monitor and evaluate the effectiveness of the program in meeting participant needs and agency and funder requirements, maintaining high program standards</li> <li>Market and promote services to referral sources, community professionals, prospective participants, family members, and others</li> </ul>
Requirements: <ul> <li>BSW (or BA in social work) from accredited institution + five years social work or related experience with older adults, adults with disabilities in social services, residential, health care systems settings or</li> <li>MSW from accredited institution + two years social work or related experience with older adults, adults with disabilities in social services, residential, health care systems settings</li> <li>Social work license in Minnesota (LSW, LGSW, LISW, LICSW)</li> </ul>
