Overview: <div><strong>$1,500 Sign on bonus</strong></div> <div>&nbsp;</div> <ul> <li>Provide advocacy, information and referral and case management to MPHA residents to enable them to live as independently as possible.</li> <li>Together with supervisor, evaluate and adjust services in the buildings as needed to ensure best outcomes for clients and residents.</li> <li>Outreach to individuals and groups within assigned buildings to ensure that residents and potential clients are aware of available services.</li> </ul>
Responsibilities: <ul> <li>Interpret and promote other VOA services and community programs to residents so that they are knowledgeable about resources available to them.</li> <li>Provide group services to residents to meet identified needs.</li> <li>Maintain case file documentation to ensure that professional standards of documentation are followed, and maintain client medical information, financial records and personal information in a confidential manner.</li> <li>Collaborate with other VOA services and community professionals to ensure that a wide network of resources is available for Highrise clients.</li> <li>Participate in case review sessions on a regular basis to help guide and strengthen social service skills.</li> <li>Participate in training opportunities to maintain and improve professional social service skills.</li> <li>Assures client privacy is maintained by being knowledgeable and following HIPAA privacy and security regulations.</li> <li>Work as many hours as needed to complete the job.</li> <li>Attendance.</li> <li>Use internal electronic health record and/or other electronic documentation systems to complete documentation as required</li> <li>Ability to respectfully interact with a wide variety of people&nbsp;</li> <li>Analytical and problem-solving skills</li> <li>Local travel required</li> <li>Sit, stand, walk, lift 25 pounds, do concentrated computer work</li> </ul>
