Overview: <ul> <li>Lead, plan, and oversee the development and implementation of activities and services with the</li> <li>Communicate, collaborate, and develop positive partnerships with the extended community to develop positive provider networks.</li> <li>Analyze information, evaluate the effectiveness, and monitor the results of the program in meeting participant, VOA, Trellis, state, and county agency requirements.</li> </ul>

Requirements: <p><strong>Education/Experience:</strong></p> <ul> <li>Licensed Clinical Social Worker (LCSW).</li> <li>1-year leadership experience.</li> <li>State of MN Social Work</li> <li>2-years&rsquo; experience in a social work role with elderly and diverse populations.</li> </ul>
