Overview: Conduct outreach and promote Community Health Worker services to individuals in a variety of locations and communities, to ensure awareness of available services.<br /><br />Conduct introductory and ongoing visits and calls to set health goals, support them in reaching the goals, and connect them with resources they need.
Responsibilities: <ul> <li>Manage a caseload of clients for 1:1 visits and follow the CHW Pilot manual and protocols in delivery of services.</li> <li>Provide, or arrangement for the provision of, CHW group services to meet identified needs.</li> <li>Lead group health sessions supporting client&rsquo;s self-management of their chronic conditions (for example: Diabetes Prevention Program, Chronic Disease Self-Management Program, Health BINGO, etc.).</li> <li>Assist and help coordinate on-site events (for example: blood pressure checks, medication management education, etc.) featuring Community Paramedics and/or Community Pharmacists.</li> <li>Support clients in seeking help if medical issues arise.</li> <li>Coach clients to get the most out of primary and specialty doctor&rsquo;s appointments to support their individual health goals.</li> <li>Attend important doctor appointments with clients.</li> <li>Use established systems to maintain files on clients served.</li> <li>Complete and maintain timely, accurate documentation and reports so that the program information is available and funder guidelines are met.</li> <li>Participate in CHW project meetings to help develop the CHW approach at VOA.</li> <li>Participate in training opportunities to maintain and improve CHW skills and documentation skills.</li> <li>Provide information concerning community resources to clients to assist people to live as independently and healthfully as possible.</li> <li>Interpret and promote other VOA services and community programs to residents so they are knowledgeable about resources available to them.</li> <li>Collaborate with other VOA services and community professionals to ensure that a wide network of resources is available for program participants.</li> <li>Assure program participant privacy is maintained by being knowledgeable and following HIPAA privacy and security regulations.</li> <li>Complete the job in the assigned hours.</li> <li>Other duties as assigned.</li> </ul> <br />Supervisory Responsibilities:<br /> <ul> <li>Works under moderate supervision.</li> </ul> Decision Making Authority/Self-Initiative:<br /> <ul> <li>Independently perform duties and carry out work assignments in conducting the operation of the business.</li> <li>Use professional discretion and demonstrate initiative to accurately and timely perform duties.</li> </ul> Professional relationships to be maintained:<br /> <ul> <li>Co-workers, building staff, community partners, local public health, and other community professionals.</li> </ul>
