Overview: <p class="ignore-global-css"><span class="ignore-global-css">Yellowstone Boys and Girls Ranch Targeted Case Managers manage all aspects of a youth&rsquo;s case while providing critical community resources to support youth and families. Targeted Case Managers provide wrap-around support for families while working collaboratively with current and prospective services to meet any youth&rsquo;s complex needs.</span></p> <p class="ignore-global-css"><span class="ignore-global-css"><strong class="ignore-global-css">Starting Salary Range:</strong>&nbsp;&nbsp;$20.39 - $22.29 per hour. Salary reflects steps 1-5 of an 11-step pay schedule. The starting salary will be determined based on the applicant&rsquo;s education and relevant experience.</span></p>
Responsibilities: <ul class="ignore-global-css"> <li class="ignore-global-css"><span class="ignore-global-css">Advocate, support, and guide youth and families in obtaining the needed services including, but not limited to, advocacy, assessing youth and family&rsquo;s needs, referring to services, creating and implementing service plans, and monitoring services of the youth and family.</span></li> <li class="ignore-global-css"><span class="ignore-global-css">Maintain contact with other service providers to ensure the unique needs of the youth and family are best met.</span></li> <li class="ignore-global-css"><span class="ignore-global-css">Research the various services offered within the community to identify services that would best meet the unique and individual needs of youth and families.</span></li> <li class="ignore-global-css"><span class="ignore-global-css">Collaborate with the youth, the youth's family, and other professionals involved to develop a treatment/service plan that addresses the specific needs of the youth including, facilitating, and leading the treatment team meetings.</span></li> <li class="ignore-global-css"><span class="ignore-global-css">Complete, update, and maintain all necessary documentation within the guidelines set by COA, Montana ARM's, Contracts, and YBGR Policy and Procedures.</span></li> <li class="ignore-global-css"><span class="ignore-global-css">Complete, submit, and verify accuracy of paperwork, including but not limited to progress notes, intake paperwork, etc., while maintaining Performance Quality Improvement (PQI) and financial viability of the program.</span></li> </ul>
Requirements: <ul class="ignore-global-css"> <li class="ignore-global-css"><span class="ignore-global-css">21 years old or older.</span></li> <li class="ignore-global-css"><span class="ignore-global-css">Criminal/Child Abuse Clearance:<em class="ignore-global-css">&nbsp;</em>Must pass criminal record and child abuse registry background check as required by the State of Montana.</span></li> <li class="ignore-global-css"><span class="ignore-global-css">Education: Bachelor&rsquo;s degree in human services related field of study. A combination of previous professional experience and formal education from an accredited university may be considered in lieu of degree.</span></li> </ul> <br /> <p class="ignore-global-css"><span class="ignore-global-css"><strong class="ignore-global-css">Required Abilities/Skills:</strong></span></p> <ul class="ignore-global-css"> <li class="ignore-global-css"><span class="ignore-global-css">Ability to relate and build rapport with youth and families therapeutically while serving as a positive role model.</span></li> <li class="ignore-global-css"><span class="ignore-global-css">Knowledge of and ability to maintain professional therapeutic boundaries and confidentiality with youth and families.</span></li> <li class="ignore-global-css"><span class="ignore-global-css">Possess the ability to work with youth and families in multiple settings, such as within the school, home, and community.</span></li> <li class="ignore-global-css"><span class="ignore-global-css">Ability to work in cooperation and collaboration with the treatment team.</span></li> <li class="ignore-global-css"><span class="ignore-global-css">Display regular, predictable, and reliable attendance at work.</span></li> <li class="ignore-global-css"><span class="ignore-global-css">Excellent active listening skills.</span></li> <li class="ignore-global-css"><span class="ignore-global-css">The ability to complete forms within an electronic Medical Health Record System typing at least 35 words per minute while using a keyboard and mouse (Familiarity with Microsoft Office 365 products is a plus).</span></li> <li class="ignore-global-css"><span class="ignore-global-css">Demonstrate sound &amp; ethical judgment and decision-making skills.</span></li> <li class="ignore-global-css"><span class="ignore-global-css">Demonstrate being self-motivated, self-disciplined, and diligent with forward thinking.</span></li> <li class="ignore-global-css"><span class="ignore-global-css">Demonstrate the ability to think of their team over self and define success collectively rather than individually.</span></li> <li class="ignore-global-css"><span class="ignore-global-css">The ability to effectively interact and communicate with others.
